Restored Chiropractic & Wellness
Privacy Policy

How we handle your information

Effective date: May 4, 2026

Restored Chiropractic & Wellness(“we,” “our,” or “us”) respects your privacy. This Privacy Policy explains what information we collect when you use restoredchiropractic.com, how we use it, who we share it with, and the choices you have.

This policy applies to information collected through our website. As a chiropractic healthcare practice, separate protections under the Health Insurance Portability and Accountability Act (HIPAA) also apply to your protected health information collected in the course of clinical care — see the HIPAA section below.

Information we collect

Information you give us directly

  • Contact form submissions: name, email, optional phone number, and the contents of your message.
  • Booking requests: when you book through our booking funnel, you are taken to our scheduling system (currently Jane App at restoredchiropractic.janeapp.com, eventually transitioning to GoHighLevel). Information you enter there — such as name, contact details, appointment type, and any health information you provide — is collected directly by that system under their privacy practices and under our HIPAA Notice of Privacy Practices.
  • Email or phone communications: any information you choose to share when you contact us by email, phone, or text.

Information collected automatically

  • Standard server logs: our hosting provider (Vercel) logs IP addresses, browser type, pages visited, and timestamps to operate the site, prevent abuse, and diagnose problems. We do not actively use these logs for marketing.
  • Cookies and similar technologies: we currently do not set tracking cookies or use third-party analytics. If we add analytics in the future (such as Google Analytics), this policy will be updated and a notice will be provided.

Information collected from animals

If you book chiropractic care for your animal, we may collect the animal's name, species, breed, age, health history, and any related information you choose to share. This is used solely to provide care. It is not personal information about you, but we treat it with the same care.

How we use your information

  • To respond to your inquiry, message, or booking request
  • To schedule, confirm, and follow up on appointments
  • To provide chiropractic, rehab, or functional medicine care
  • To improve our services and the operation of this website
  • To comply with legal and regulatory obligations

We do not sell your information. We do not share it with advertisers or data brokers.

Third-party services we use

We use a small set of third-party services to run the practice and the website. Each has its own privacy policy you can review:

  • Jane App (online scheduling and patient management): when you book an appointment, you are routed to Jane and any data you provide is held by Jane.
  • GoHighLevel (CRM and future scheduling): we are transitioning some scheduling and patient communication to GoHighLevel. When that transition occurs this policy will be updated.
  • Vercel (website hosting): hosts and serves the website. Receives standard request logs as described above.
  • Google Maps: an embedded Google Map appears on the contact page. Google may set cookies and collect browser information when the map loads.

We may also share information when required by law, in connection with a legal proceeding, or to protect the safety of patients or staff.

Affiliate links and third-party products

Some links on our Trusted Partners page are affiliate links — meaning we may receive a small commission if you purchase through them, at no extra cost to you. We only recommend products we personally use and trust. Any data you provide on those third-party sites (Juice Plus+, Purity Coffee, Fullscript, etc.) is collected and managed by them under their own privacy policies.

HIPAA and your health information

Restored Chiropractic & Wellness is a covered healthcare provider under the Health Insurance Portability and Accountability Act (HIPAA). Information you provide in the course of clinical care — including health history, examination findings, treatment records, and billing — is protected health information (PHI) and is governed by our Notice of Privacy Practices, which is provided at your first visit and is available at the clinic on request. Our website Privacy Policy supplements that Notice for the digital side of the practice but does not replace it.

Please do not include sensitive medical details in our website contact form. For specific health questions, contact us by phone or schedule a discovery call so we can talk securely.

Data retention

We retain contact form submissions and email correspondence long enough to respond and follow up. Booking and clinical records are retained for the period required by North Carolina chiropractic regulations and applicable healthcare laws (typically several years after the last patient encounter). We delete non-essential information when it is no longer needed.

Your rights and choices

You can:

  • Ask what information we have about you and request a copy
  • Ask us to correct information that is inaccurate
  • Ask us to delete information we hold about you, subject to our legal record-keeping obligations
  • Opt out of any non-essential email communications

Residents of California, Virginia, Colorado, Connecticut, Utah, and other states with consumer privacy laws have additional rights. To exercise any of these rights, email patrick.rebadow.dc@gmail.com or call 919-578-1990.

Children's privacy

Our website is not directed to children under 13, and we do not knowingly collect information from children under 13 through the website. We do treat children as patients in the clinic — that information is handled under HIPAA and provided with consent from a parent or guardian.

Security

We use reasonable administrative, technical, and physical safeguards to protect information we hold, including encrypted connections (HTTPS) on the website and secure storage with vetted providers. No method of transmission over the internet is 100% secure, so we encourage you to share sensitive information by phone or in person rather than through web forms.

Changes to this policy

We may update this Privacy Policy from time to time. The effective date at the top of this page indicates when the latest version took effect. Material changes will be flagged clearly when they occur.
